Job Description:
Personal Care

Care Coordinator - Vacancy no longer available
Job Description: This job is no longer available (please do not apply).
- To assist in the day-to-day operation of the Home Care Service business, involving direct line management for a team of Home Care Workers. This includes the management of their weekly workload ensuring all calls and working patterns are coordinated to the needs of the company, customer and care workers. Also making sure that services are delivered in line with their purchase order/private contract and that staff receive regular quarterly supervision.
- To participate as required in the multi-disciplinary assessment of Service User needs and the subsequent planning of Service User Care and including the use of the People Planner rostering system.
- Undertake client care reviews on a continuous basis and adhere to CQC guidelines.
- Completion and distribution of the care worker rotas are completed on a weekly basis to all appropriate parties. I.e. you will need to make sure that rota’s are available for collection or distributed accordingly.
- Audit all care records such as financial transactions, daily logs and medication records
- Provide dedication, leadership and motivation to your team and work colleagues and ensuring respectful relationships are maintained.
- To participate in Staff, Team and Quality Management Review Meetings as directed by the Registered Manager.
- To ensure that on call duties are in place as required by the Orchard Homecare Services Ltd to ensure a 24/7 service is provided.
