User Assessment/Risk Assessment
Prior to commencing your care package one of our Senior care workers will carry out a visit to undertake
a risk assessment. NB: In emergency situations a provisional assessment may be made prior to care commencing with
a full assessment being completed shortly after
Care Plans
Care plans are, in normal circumstances, prepared prior to care commencing. Plans are prepared with
either the service user themselves or the Service User’s Social Worker (Circumstances dependant).
NB: In emergency situations a provisional plan may be made prior to care commencing with a full plan being completed
shortly after.
Once created the Care Plans are reviewed regularly and updated to reflect any changes required..
A copy of the Service Users Care Plan can be found in file which is kept in the home of the Service
User.
Service users and their relatives are always welcome to contact the office if they have any concerns
about their care or care plan
